The Behavioral Health Care Manager is the primary point of contact for patients who are enrolled with April Health and responsible for coordinating the patientβs care with the patient's primary care provider and consulting psychiatrist. They are also responsible for tracking the patientβs symptoms with validated measurement tools, and directly meeting with the patient to deliver evidence-based treatments.
